Fall Prevention Month - A Message from the Simcoe Muskoka District Health Unit

November is here, and you know what that means—it's Fall Prevention Month

Get ready to have some fun with the SMDHU's Fall Prevention Trivia Game! Hyperlinked is your go-to game that’s perfect for November and can be used all year long!

We are also excited to share the latest videos featuring older adults across Simcoe Muskoka sharing their stories about how they stay active, eat well, and meet their social and emotional needs. We hope these videos inspire new ideas for others in our community.

Information and Resources

Accessibility and Functional Needs Registry

Do you or a loved one have medical, access, or functional needs that would place you at higher risk during an emergency event? It is important to plan ahead for disasters as resources can be very limited during an emergency. If you are on oxygen, have a physical disability that would make it difficult to evacuate or have other medical needs, you are strongly encouraged to fill out the Tay Township Accessibility and Functional Needs Registry as part of your Emergency Preparedness Plan.

Participating in the Accessibility and Functional Needs Registry is completely voluntary. Information submitted is strictly confidential and will only be used in the event of an emergency. The online form is available at tay.ca/AFNR or available for pick-up at the Municipal Office located at 450 Park Street, Victoria Harbour during our regular business hours of Monday to Friday from 8:30 a.m. to 4:30 p.m.

If you require additional assistance with this form please reach out to Jenny via email at jduffy@tay.ca or call 705-534-7248 x262.
